We are seeking a compassionate and dedicated Community Health Worker (CHW) or Certified Peer Support Worker (CPSW) who is interested in making a change in the community and joining our team. The selected candidate will provide support services to clients using Trauma-Informed Care, Harm Reduction, and Client-Centered Approaches.
This role involves working closely with clients to enhance their well-being by offering education and coordination of care aimed at improving social determinants of health and will play a crucial role in fostering positive health outcomes within the community. UNM OCH Community Health Intensive Care Management role involves working directly with assigned clients and addressing their unique needs, which can consist of substance use disorder, co-occurring disorders, criminal justice involved and / or experiences homelessness.
The CHW will work to support these individuals by addressing their unique needs and providing resources and guidance to improve their overall quality of life.As a CHW / CPSW, you will work under the direct supervision of the Program Manager with the Intensive Case Management Program. You will play a crucial role in fostering positive health outcomes within the community. The selected individual will engage with clients in a community-based setting, community, correctional facility, and / or conduct home visits to ensure comprehensive support.
Conditions of Employment
- Possession of a valid New Mexico driver's license is a requirement for this job.
- Must pass a pre-employment criminal background check.
- Fingerprinting, and subsequent clearance, may be required for this position depending on assignment.
- May be required to obtain a Community Support Worker (CSW) certification within six months of hire.
- Employees who provide services or work in patient care or clinical areas are required to be in compliance with the University's influenza vaccination requirement.
- Successful candidate must submit to post-offer, pre-employment physical examination and medical history check.
Minimum Qualifications
High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
