Job Summary
The Community Health Worker is responsible for helping patients and their families navigate and access community resources, providing disease care and / or prevention education, providing social support and informal counseling, and encouraging patient engagement in reaching goals and adopting healthy behaviors. Advocate for individuals and community health needs.

This position will work closely with community benefit providers, social services team, care coordinators, providers, and other team members to ensure quality outcomes for patients
Physical Capabilities
Work is of medium demand; walking, sitting and standing most of the time while on duty. Occasional lifting of patients with assistance is required. Adequate hand / eye coordination and fine motor skills required. Talking and hearing is essential in instructing and teaching others. Near visual acuity required to work with charts and records and to observe patients.
Environmental Working Conditions
Works in a well-lighted, heated and ventilated building. Required to do home visits, therefore, dependable transportation is required. Hours may vary to accommodate needs of the patients. Exposure to blood borne pathogens is a risk.
