The Registered Dietitian supports the mission of People’s Community Clinic by providing direct service nutritional care for PCC patients in an efficient, effective, and timely manner.
The starting Salary range for this position is $55,000 - $60,000 annually.
Responsibilities :
Provide direct service nutritional care for PCC patients according to established protocols and physicians’ orders.
Perform age-appropriate nutritional assessment.
Take nutrition history, record chart data, and service data as per protocol, established procedure, contract requirements, and accepted standards of clinical practice.
Assess nutritional status and develop a nutrition care-plan that reflects nutrients needs determined from the information gathered, and in consultation with the attending physician.
Provide patients with in-person or virtual counseling concerning nutrition recommendations, general health and wellness, physical activity, and nutrition management of the diagnosed condition.
Provide patients with counseling regarding follow-up treatment and patient inquiries.
Provide referrals to social services and other support staff as deemed necessary.
Complete designated chart review on schedule and according to procedures.
Coordinate and Oversee Group Nutrition Programming :
Participate in program planning, development, evaluation, and implementation of group programming.
Develop and monitor participation.
Participate in group nutrition education and support other team members, assisting with regular and cooking classes.
Co-facilitate group medical visits (e.g., Integrative Pain Management Program).
Maintain standards of proficiency, service quality, efficiency, and effectiveness in all task areas :
Achieve and maintain proficient levels of nutrition and dietetic knowledge and skills. Keep continuing education requirements up-to-date.
Provide direct care according to schedule.
Maintain professional affiliation and community relationships with other local entities.
Adhere to HIPAA guidelines.
Participate in program planning, development, evaluation and implementation activities as required.
Work with the Nutrition Services Program Manager to update protocols.
Create new and review existing nutrition education materials for accuracy, utility, and appropriateness.
Serve as a nutrition resource to the assigned program service area to ensure required follow-up, to identify emerging patient needs, to ensure nutrition education is complementary to direct service component.
Produce written reports and documentation as requested.
Participate in meetings, task groups, and activities designed to promote communication; identify and resolve problems, devise work plans, and establish program objectives.
Attend and participate in team meetings and staff meetings as scheduled.
Demonstrate understanding and support of the PCC mission and program objectives by participation and support of work plans devised to enact the mission and achieve service objectives.
Work openly and cooperatively with the medical director, team leader, and other employees to achieve the highest standards of operational efficiency and service effectiveness.
Qualifications :
Education : B.S. or B.A. or higher degree with graduation from an accredited university.
Experience : Minimum two years related experience serving diverse patient populations in a community health setting, specifically with regard to diabetes, hypertension, hypercholesterolemia, and obesity.
Knowledge, Skills, and Abilities :
Understanding of the theory and practice of nutrition and dietetics; assessing the nutritional status of patients using physical, clinical, and biomedical data; public and community health service practices; generally accepted professional and ethical standards of patient-centered care.
Ability to conduct brief screenings or comprehensive assessments of nutrition-related data for each patient. This involves developing tailored nutrition care plans based on factors like height, weight, unintentional weight changes, appetite changes, medical and social history, and clinical findings. Key skills include charting, reviewing medical history and laboratory results, maintaining accurate records, and effective time management, along with strong problem identification and resolution abilities.
Ability to apply knowledge gained through nutrition program, especially related to diabetes, hypertension, hypercholesterolemia, and obesity.
- Ability to establish and maintain effective and consistently cordial working relationships with physician(s), other employees, team leader, executive director, and patients;
- Able to manage time effectively to ensure efficiency and timely follow through on tasks; able to accommodate changes in the workplace (e.g. procedures, scheduling, staffing, etc.); able to work effectively under pressure; maturity, flexibility, stability, initiative;
Interest in and commitment to the provision of quality healthcare to persons at or near the poverty level.
Experience co-facilitating group medical visits and classes.
Experience in presenting in group settings.
Bilingual in English-Spanish and familiarity with Hispanic population culture and eating habits, preferred.
Certificate / License : Applicant must be a Registered Dietitian through the Academy of Nutrition and Dietetics and a Licensed Dietitian by the state of Texas.
