Job Description :
ESSENTIAL JOB FUNCTIONS
- Conducts Michelle P. Waiver assessments and reassessments, including a face-to-face visit with the individual in the home within the 14-day deadline, or other deadline, as established by state criteria.
- Completes initial assessments for individuals that are new to the Michelle P. Waiver. Provides information on the delivery of service type and directs individuals on how to find available case management agencies.
- Documents responses from individuals or their caregivers / guardians based on a fifteen-page assessment tool designated by the state. The document covers all aspects of an individual’s health, ability to complete activities of daily living, neurological and emotional status, social supports, and physical environment.
- Obtains consents and completes forms assigned by the agency and the state. Ensures documents are shared with the individual in a secure manner. Ensures documents are shared with medical records staff.
- Completes information notes and assigned documents in the electronic medical records system, within given deadlines.
- Arranges for submission of the MPW evaluation to the Quality Improvement Organization, which determines the Level-of-Care approval or denial, and the assessment and / or reassessment to the respective MPW provider upon approval of the Level of Care.
- Communicates with individuals, their families, and case managers on the status of their assessment throughout the evaluation process.
EDUCATION
Registered nurse or social worker (defined by regulation as a person with a Bachelor’s degree in social work, sociology, psychology or a related field as approved by Medicaid)EXPERIENCE
Experience preferred but not required.Working knowledge of the MPW and eligibility criteria.Reliable transportation for frequent travel.PHYSICAL DEMANDS
Position has no unusual physical demands.Position requires travel to offsite locations and may require walking or standing, stooping or bending.Position requires extensive work in client’s homes, occasionally entailing modestly unpleasant situations.Time Type : Full time
