A TEAM OF DIFFERENCE MAKERS :
We seek an Outreach Liaison to increase patient engagement with Author Health services and serve as one of the first points of contact for patients and their caregivers. You will spend time building relationships with community members, educating them on the impact of our services, and getting their initial visit scheduled with the clinical team.
WHAT IS YOUR SUPERPOWER?
- Meet patients in their homes, at primary care officers, other community settings, or telephonically / virtually to form relationships, educate on Author Health services and
- schedule intake visit with Author Health Provider.
- Conduct door-to-door canvassing to engage hard-to-reach patients
- Respond to Health Information Exchange (HIE) alerts (real-time notifications related to hospital admissions, transfers, or discharges) and meet with patients and their caregivers in the hospital or other care facilities
- Develop and implement community outreach programs and events to increase awareness and engagement rates
- Build partnerships with local organizations, community leaders, external providers and brokers to expand the reach of outreach efforts
- Keep accurate records of all patient interactions in the engagement phase
- Assess patients’ use of technology and assist conducting virtual visits with other providers on the team
- Identify and remove barriers to engagement with necessary resources and supports
- Communicate and provide education on the value of Author Health to patients and their caregivers
- Successfully meet and exceed monthly engagement targets
WHAT WE ARE SEEKING :
High school diploma / GEDBilingual (Spanish-English or Haitian Creole-English)Strong organizational skills and verbal / written communication skillsPossession of driver’s license and car At least 1 year of experience directly related to the duties of the positionPassion for, and experience working with, patients with behavioral health conditionsPassion for, and experience working with, seniorsStrong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse communityKnowledge of community agencies and resourcesWorking knowledge of multi-system outreach programs related to health care deliveryAbility to communicate medical information to health care professionals, care coordinators, and patientsAbility to organize resources and effectively prioritizePreferred :
Associate’s or Bachelor's degree in health / human services or other relevant field preferredBilingual (Spanish-English)Direct-to-consumer sales or patient engagement at a health plan, medical facility or another healthcare settingWHAT WE OFFER :
