About the Role :
Tidewell Hospice is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.
Key Responsibilities :
- Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
- Provide individual and family counseling, crisis intervention, and bereavement support.
- Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
- Assist with financial resources, Medicare / Medicaid, and long-term care planning.
- Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
- Support patients and families with advance directives and memorial planning.
- Educate the community on hospice care.
- Participate in an on-call rotation.
Qualifications :
Varied schedule : Provide 64 hours monthly; Monday - Friday 8am - 5 pm. May assist hospice houses, home, ALF or LTC teams.
