About Houston-Galveston Area Council

What will I bedoing?

ü Deliver core programs of the Houston-Galveston Aging & Disability Resource Center.

ü Provide Options Counseling to non-Medicaid nursing facility residents relocating to the community, providing decision support as the resident makes informed choices about potential services and supports in the community.

ü Coordinate respite activities for family caregivers.

ü Conduct outreach on respite services and Aging & Disability Resource Center services.

ü Coordinate activities of the Housing Navigator program, including maintaining a housing inventory of affordable and accessible housing options, attending housing coalition meetings, and maintaining relationships with housing developers.

ü Help connect individuals living with a disability, older adults, and family caregivers to long-term services and supports through the Aging & Disability Resource Center toll-free phone line.

ü Conduct data entry and maintain proper documentation of program activities.

ü Deliver reports to the program's leadership team and to program funder.

KeyQualifications

ü Skilled in client advocacy

ü Resourceful navigator of community resources and Texas Health and Human Services service array.

ü Strong written and verbal communication skills.

ü Proficient in Microsoft Suite.

Do you have...

ü Bachelor's degree in an applicable academic discipline orrelatedfield of study

ü At least 2 years of experience working with local government, nonprofit programs, school, or in job-related duties

Preferred Qualifications

ü Bilingual in English and in Spanish

ü Certified in CIRS-A / D through Inform USA
