Our Residential Program is looking for a dynamic and detail oriented LPN Case Manager to support the Agency's 24 hour, Residential Program as regulated by the Department of Developmental Services (DDS) by providing nursing oversight of assigned caseload ensuring program individuals receive premier services and achieve optimal health. Responsible for providing daily and / or as needed nursing case management to individuals including but not limited to, coordination of healthcare appointments, regular monitoring of medications and Medication Administration Program (MAP) compliance, oversight of annual health and wellness screenings, coordination of care during hospitalizations and assisting managers with the discharge process, providing medical / health updates to the individual's support team, providing staff education / trainings relating to MAP, and ensuring complete and accurate medical / healthcare documentation and follow up. Additionally, this position will provide nursing triage and assessments during regular hours and during after-hours rotation.

CASE MANAGEMENT

  • Monitor the general health and well-being of program individuals and engage in appropriate follow up to ensure individual health needs are met.
  • Conduct nursing assessments on individuals as needed.
  • Conduct routine healthcare audits to ensure individuals receive all recommended preventative care in alignment with DDS Adult Screening Recommendations.
  • Assist with healthcare communication between healthcare providers, families, DDS, and other stakeholders, as necessary.
  • Assist program management in preparation of medical components of Individual Support Plans (ISP) meetings, participate in healthcare-related discussion during meeting.
  • Collaborate with program management to ensure healthcare orders, Medication Administration Records, healthcare protocols / guidelines, and all other documentation remain in compliance with all applicable laws, policies, and regulations, at all times.
  • Collaborate with program management to ensure appropriate stock of medication, and maintain up-to-date Medication Treatment Plans and Supportive and Protective documents, when indicated.
  • Collaborate with program management to ensure individual Healthcare Records (HCRs) are accurate and remain up-to-date in the Home and Community Services Information System (HCSIS).
  • Collaborate with program management to prevent falls via fall prevention program.
  • Collaborate with program management during hospital admission and discharge process and coordinate care to ensure individuals receive appropriate healthcare follow up and advocacy.
  • Identify potential need in creation of healthcare proxy, indication for change in medical code status, or beneficial services such as palliative or hospice care.
  • Provide training and education to staff specific to healthcare needs and MAP-related items.

GENERAL NURSING DUTIES :

  • Provide after hours on-call triage and / or telehealth to provide medical support to individuals to reduce unnecessary hospital and emergency room visits, (Non-relief positions).
  • Participate in the admission process for new referrals into the residential homes.
  • Conduct periodic infectious disease testing as needed.
  • COMPLIANCE RELATED FUNCTIONS :

  • Follow all MAP requirements effectively and efficiently.
  • Conduct routine audits reviewing for compliance with MAP including but not limited to : healthcare provider orders, Medication Administration Records, pharmacy labels, countable medication book, and medication disposal records.
  • Conduct routine audits on individual healthcare related documentation to ensure complete / accurate nursing assessments and healthcare records, including but not limited to : healthcare provider encounter forms, lab / diagnostic test documentation, hospitalization records, and medical progress notes.
  • Adhere to all Human Rights.
  • ADMINISTRATIVE RELATED FUNCTIONS :

  • Provide staff education and training consistent with LPN / RN role, as indicated.
  • Conduct MAP classes, per MAP annual requirement or as assigned (RN only).
  • Act as a MAP Consultant to advise on specific medication issues (exception for LPN and Relief RN).
  • Assist with drafting healthcare policies, protocols, and guidelines.
  • Support Program managers on annual health and wellness screenings.
  • Attend Nursing Department Program and staff meetings as required.
  • QUALIFICATION

  • Current unrestricted licensure in Massachusetts as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) required. (Bachelor's degree preferred).
  • 2 years' experience in nursing (preferred).
  • MAP Trainer Certification for RN's (preferred).
  • 1-year experience working with intellectual / developmental disabilities population preferred.
  • Skill obtained through First Aid / CPR (Healthcare Provider), medication administration, and site-specific certifications.
  • Ability to perform nursing skills and critical thinking within the LPN / RN scope of practice.
  • Ability to maintain reliable transportation and travel to local areas to include but not limited to : Fall River, Somerset, Swansea, Westport, Taunton, and Mansfield.
  • Ability to maintain confidentiality and HIPAA standards.
  • Ability to work independently and as a team.
  • Ability to work and communicate effectively.
  • A qualifying criminal background check and MVR check required.
