POSITION OVERVIEW

A Care Manager provides direct care to individuals served with behavioral health needs, including supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need through a collaborative triage, assessment, planning, and communication process. The position r equires traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.

JOB REQUIREMENTS :

  • Must have a Bachelor's Degree in Human Services or a related field of study toward Human Behavior / Science.
  • Must be able to lift and carry up to 50lbs of dead weight
  • Must have excellent interpersonal skills
  • Valid Florida Driver's license, reliable personal transportation, and insurance.

    • POSITION RESPONSIBILITIES

  • Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
  • Complete all clinical and administrative documentation based on established timelines as per program requirements.
  • Communicate effectively and maintain cooperative relationships with internal and external resources.
  • Develop service plans, treatment plans, and / or aftercare plans with clear goals and objectives as per program requirements.
  • Maintain efficient workflow management and organization as per program requirements.
  • Maintain proficient quality of clinical documentation as per program requirements.
  • Other duties as assigned or needed as per program requirements.

    • WORK ENVIRONMENT :

  • Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe supportive environment.
  • This is a 60-bed Adult Crisis inpatient served care facility.
  • Office setting within the Crisis center
  • Must be capable of lifting 50 pounds of dead weight alone.
