Job Summary :

The pharmacist will largely support primary care clinics at VUMC. The pharmacist will engage with patients, providers, and the care management team (nurses and social workers) to ensure effective, safe, and cost-effective drug therapy in a population health model.

Responsibilities :

• Addressing patient referrals from providers
• Performing comprehensive medication reviews and medication reconciliation related to transition of care episodes
• Assisting with chronic disease management, including titrating medications, recommending and implementing therapy modification, monitoring medications and conditions, and coaching patients through lifestyle modifications
• Dispelling care gaps and remedying reported medication issues with a goal to graduate patient or assist in handing off to Complex Care Team
• Optimizing complex medication regimens including addressing polypharmacy concerns and deprescribing
• Medication coverage, assisting provider request on department systems.
• Formulating responses to drug information questions
• Providing medication education for patients, providers, and colleagues
• Connecting patients, families, and providers to community resources
• Participating in quality improvement initiatives.
