Centralized Plan Coordinator

In this role, you will collaborate with the person receiving services as well as their support team to develop and revise their goals. The Centralized Plan Coordinator ensures consistent implementation and documentation of all aspects of the person's program plan. This position primarily oversees the planning of services based on person centered planning concepts that are delivered to each person receiving services through Arc of Onondaga.
