Riverside PostAcute Care -

Riverside PostAcute Care is looking for a Registered Dietitian with Skilled Nursing experience.

P OSITION SUMMARY / PURPOSE

To provide high-quality medical nutrition therapy and work as an active member of the multidisciplinary team. To be a resource on nutritional information to physicians and staff and represent Food and Nutrition Services throughout the District.

We are looking for someone who has :

Working knowledge of the Minimum Data Set (MDS), with experience completing MDS entries for Section K

Experience in formulating individualized plans of care (IPOC) for skilled nursing facility (SNF) patients

Ability to collaborate with nursing staff on monthly weight monitoring

Ability to participate in interdisciplinary team conferences

Experience in providing long-term enteral nutrition support

Ability to work with nursing and medical staff to assist with wound healing interventions

Responsibilities :

Assesment of the clinical nutrition of the residents and oversight of the kitchen and food service

Evaluate the nutritional care of residents and document in the medical record

Recommend dietary interventions according to the status of the residents

Evaulate changes to the facility's menus for nutritional adequacy
