Responsibilities

Job Summary / Purpose

The Patient Navigator will act as the key non-clinical contact that identifies needs and provides information about services and processes.

Essential Key Job Responsibilities

  • Guide patients through and around barriers within the healthcare system.
  • Maintaining communication with patients, families, and health care providers to monitor patient satisfaction.
  • Direct patients to resources and manage information.
  • Coordinating screenings and appointments.
  • Arranging transportation.
  • Facilitating follow-up services.
  • Facilitating financial support and helping with paperwork.
  • Perform other duties as assigned.
  • Qualifications

    Required Education and Experience

    Bachelor’s Degree

    Two years of related experience in a healthcare environment.

    Required Licensure and Certifications

    None

    Required Minimum Knowledge, Skills and Abilities

    Strong customer service skills.

    Knowledge of medical terminology.
