Hospice RN
Responsibilities :
- Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
- Admissions of patient for services and development of the initial patient / family Plan of Care
- Coordination of services with the patient, family, and hospice interdisciplinary team members
- Ongoing clinical supervision of Licensed Practical Nurses and Home Health Aids
