A company is looking for a Palliative Care Nurse Navigator.Key ResponsibilitiesConduct comprehensive symptom assessments and facilitate end-of-life care discussionsEngage with members to address clinical issues and manage escalations efficientlyCollaborate with healthcare teams and support members with serious illness throughout their care journeyRequired QualificationsBachelor of Science Degree in NursingCompact unrestricted registered nurse (RN) license; New York, Illinois, and / or California licensure preferredAt least 5 years of nursing experience, including 3 years in outpatient palliative care or home hospice for oncology patientsCertification as a Certified Hospice and Palliative Nurse (CHPN) or oncology-related certifications preferredAbility to work flexible hours, including occasional Saturdays and up to 2 holidays per year
