Your Mission :

Reporting to the Behavioral Health Program Coordinator, the Behavioral Health Case Manager works collaboratively with all CASL program departments to assess and address the behavioral health needs of entire households. This role involves visiting clients of all ages, conducting interviews, documenting assessments, and developing service plans to help clients overcome challenges and achieve their goals.

Your Impact :

As a Behavioral Health Case Manager you will :

Key Responsibilities :

Client Advocacy and Assessment (70%) :

  • Facilitate regular interdepartmental team meetings to implement the Client Advocacy Unit Model throughout the whole agency.
  • Develop and maintain relationships with community partners to connect clients with additional services / programs.
  • Conduct behavioral health intake with enrolled clients and re-assess the needs to understand the Social Determinants of Health affecting them or their family's behavioral health wellbeing.
  • Evaluate treatment options and create comprehensive service plans for clients to address their identified needs and goals, and / or collaborate with other clinical or medical professionals.
  • Work with individuals, couples, groups, and families to help address behavioral, emotional, mental, or social issues through case management, crisis intervention, community support, and / or counseling / therapy services under supervision of clinician / Qualified Mental Health Professional.
  • Hold quarterly and ad hock stakeholder meetings with individuals served and community stakeholders to obtain feedback.
  • Coordinate service delivery with the individual's primary care provider, care coordination entity, managed care entity, and / or other external entities providing care and services to clients.
  • Ensure individuals receive referrals for substance-related and addictive disorder treatment services, as needed.
  • Conduct community outreach and education to increase staff and community members' awareness towards behavioral health.
  • Ensure confidentiality of information is maintained appropriately by following CASL policies / procedures as well as the State of Illinois regulations.

Administrative Responsibilities (30%) :

  • Maintain client records and ensure timely clinical documentation of all sessions.
  • Operate in compliance with all applicable laws, regulations, and CASL policies.
  • Collaborate with the Behavioral Health Program and Marketing and Community Department on mental health media campaigns and website updates.
  • Conduct quarterly quality assurance reviews of client records and clinical care.
  • Ensure compliance with grant requirements and internal policies.
  • Participate in professional development opportunities to maintain mental health-related licensure / certification.
  • Manage data using Salesforce and Electronic Health Record Systems.
  • Must Haves - To thrive in this role, you should have :

  • Bachelor's degree in Psychology, Social Work, or a related field.
  • Bilingual proficiency in spoken and written English and Chinese (Cantonese and / or Mandarin)
  • Mental Health-related license / certificate to practice in Illinois or eligibility to obtain one, (e.g., LMFT, LCPC, LCSW, LSW, LPC)
  • Proficiency in spoken and written English
  • Excellent communication skills and conflict resolution abilities.
  • Proficiency in Microsoft Office and G-suite, or willingness and ability to learn
  • Nice to Haves

  • Previous experience in case management and counseling is preferred.
  • Basic knowledge of Salesforce is a plus
  • Physical Demands :

  • Frequent sitting, standing, bending, and walking for extended periods.
  • Work Environment and Schedule :

  • Work hours are between 8 : 00 am to 7 : 00 pm in 8-hour shifts, Monday through Friday, with at least one rotational Saturday a month.
  • Flexibility to work after regular business hours when necessary, including weekends and evenings.
  • Service delivery may occur in clients' homes or other community-based settings as needed.

    Compensation& Benefits :

    This position offers a salary range of $57,000 - $70,000 annually , reflective of experience and qualifications
