Qualifications :
Illinois : Bachelor's degree in Social Work; Master's degree preferred.
Current State Licensure in Social WorkIllinois : LSW or LCSW
Minimum of one year's experience in health care or hospice settingMaintains a current CPR certificationValid driver's license and proof of insurance is requiredHOURS : Monday-Friday 8 : 30AM-5PM
On-Call Expectations : 2-3 times / month
Location : Barrington, IL.
Salary : $62,000 - $78,000 Annually - Offer Based on Years of Experience

Performs the psychosocial assessment of patients and families / caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient's identified residence and assistance when this is not safe, and another plan is requiredDemonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently providedCounsels patient and family / caregivers and staff as needed in relationship to stress, and other identified coping difficultiesProvides accurate and timely documentation of patient services to reflect the plan of careProvides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient / family needsServes as liaison between patients and families / caregivers and community agenciesTEXT 7979 to 847-416-8078 to APPLY!
