General Job Description
This position is responsible for the healthcare coordination services to Community Based Services participants of Deborah's Place, through the provision of assessments, referrals, facilitating access to healthcare, advocating on the behalf of participants for improved healthcare, and the development and facilitation of health education activities.
Job Duties and Responsibilities
Health Services Care-Coordination
- Establish positive, supportive relationships with participants, while identifying and applying appropriate role definition and skilled boundaries.
- Conduct initial health intake assessments with participants.
- Assist participants in accessing health related services, including but not limited to; obtaining a medical home ( Primary Care Provider - PCP), providing instructions on navigating use of the medical home, overcoming barriers to obtaining needed medical care , ensuring access to dental care, eye care and other specialty needs and other health services.
- Assist participants in enrolling, maintaining, and navigating health insurance and medical benefits.
- Motivate participants to actively engage in working on their health goals.
- Work collaboratively and effectively within the Clinical Services Team to ensure effect ive health services planning, record keeping, referrals, crisis intervention and advocacy for participants.
- Document participant service data and referrals in an effective manner while adhering to policies and procedures in place.
- Work with the participants ' Care-Coordination Teams of PCP's, external Managed Care Organizations ( MCO's), and other healthcare networks to effectively manage their health conditions.
- Plan monthly health education activities via zoom or in person.
- Create and implement CBS Health Services monthly newsletter.
- Attend doctors' visits with participants upon their request / needs.
- Meet with participants in their homes upon participants' request / needs.
- Continuously expand knowledge and understanding of community health resources available for participants.
Staff Support
Provide health education to Deborah's Place Clinical Services staff as needed.Act as a resource to other staff regarding the healthcare needs of participants 'and healthcare policies and procedures. Develop health care resources.Other
Maintain required professional knowledge and job skills. Attend and participate in required educational programs and staff meetings.Represent Deborah's Place mission and values to participants, visitors, and others.Perform other duties assigned.Position Qualifications and Requirements
Education : Bachelor's degree in public health or related field, Masters preferred.
Job Experience : At least 1 year of experience in health services or a health-related setting.
Special Knowledge and Skills : Knowledge of Medicaid and other mainstream benefits; knowledge of care coordination models, experience in navigating the healthcare system for people with little or no income; experience working with people who have multiple barriers to healthcare; effective written and oral communication skills; computer literacy including experience with client service tracking software; valid Illinois Driver's License; conflict management and crisis intervention
