Responsibilities

Team Select Home Care is looking for a Director of Nursing to join our Team Select family. The Director of Nursing oversees all patient care services, clinical personnel, and the financial performance of the agency. The DON strives to provide the highest quality of patient care, people development, and performance outcomes in a patient-centered environment. The Assistant Director of Nursing and Intake / Schedulers report directly to the DON. Be a resource person to clinical staff when ADON is not available and help ensure oversight of patient and personnel assignments, patient care, and referrals. Will also help hire, evaluate, and term personnel. Help institute, interpret, and document disciplinary action. The DON reports to the Divisional Director of Clinical Operations.

• Guide and develop patient care, personnel requirements, competency skills.
• Understand the financial and business side of operations to meet budget.
• Consult with the Divisional Director of Clinical Operations in determining a staffing pattern which will accomplish shared goals and promote a maximum level of use of patient care personnel while promoting a viable financial environment.
• Ensure that all case conferencing efforts for patients are coordinated addressing high-volume, high risk and problem prone cases along with care risk type patients.
• Meet corporate goals for metrics on 5 Star, VBP, scorecards and financial metric goals as established with the Divisional Director of Clinical Operations.
• Ensure completion of standardized testing, accurate documentation and staff knowledge / completion of best practices.
• Ensure that coordination of care and documentation occurs between staff and patient / family / representative and facility and other outside agencies.
• Investigate, resolves and report any problem relating to patient care and employee well- being.
• Attend relevant continuing education programs as assigned by Divisional Director of Operations and shares information with staff.
• Oversee the Agency’s ongoing Quality Assurance, Performance Improvement (QAPI) program. Develop / implement the QAPI plan, and the identification of goals related to patient outcomes and coordinating, participating, and reporting of activities and outcomes results.
• Assist ADON in conducting patient care conferences, staff meetings and documentation. Participate in community education projects.
• Perform other tasks as assigned.
