Registered Nurse Care Manager

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.Key ResponsibilitiesEvaluate member needs and barriers to care, recommending plans for optimal outcomesDevelop and coordinate ongoing care plans in collaboration with providers and community resourcesMonitor member status and progress towards care plan goals, revising plans as necessaryRequired QualificationsDegree from an Accredited School of Nursing or a Bachelor's degree in Nursing2 - 4 years of related experienceRN - Registered Nurse - State Licensure and / or Compact State Licensure required
