Location : West Allis, WI

Pay : $82k - $88k

Schedule : M - F : weekly productivity expectation. Office hours 8 AM - 5 PM

FTE : 40 hours per week

Our client is a home health organization seeking an RN for a direct hire opportunity!

What you get to do :

Under the physician's order, admits patients eligible for home care services within 24-48 hours.

Assess and evaluate patient needs / problems, identify goals with patients.

Report patient status and need for other disciplines.

Develop patient care plan that addresses identified patient problems and goals What you need to succeed :

State professional Nurse License

One year experience as a home care professional nurse

Ability to assess and document patient needs and formulate individualized patient care plans

Proficient clinical skills and excellent communication skills

Automobile to be used for work, current driver's license, good driving record and proof of insurance What's in it for you :
