Description

Job Overview :

The Youth Street Outreach Case Manager will provide case management for homeless, runaway, and at-risk youth. In addition to direct client services, this position is also responsible for community outreach including delivering school presentations, staffing resource tables at school and community events, building relationships with school counselors / social workers, organizing / participating in drop-in services, and serving in an on-call rotation for the Safe Place network in the Kansas City Metro Area and Johnson County.

Responsibilities / Duties :

  • Works with coworkers, supervisors, and staff from other departments to solve problems and improve processes
  • Responding to outreach calls from runaway / homeless / street youth. Meeting with youth at community locations, including 2 agency drop-in centers, to provide advocacy, referrals, and community resources as needed
  • Building and maintaining partnerships with community agencies, schools, and other youth-serving contacts to spread awareness of youth homelessness and available resources
  • Planning & conducting activities & workshops for runaway and homeless youth in the community (drop-in centers, libraries, schools)
  • Participating in required staff development trainings, staff meetings & community youth-oriented coalition meetings
  • Document all significant events and encounters. Enter data into databases in a timely manner
  • Other duties as assigned
  • Bending, stooping, walking, standing, sitting and reaching is required. The team member must be able to lift 25 lbs.
  • Be able to travel between locations for meetings and training throughout the local community

Qualifications

Qualifications :

  • Associate degree or equivalent from a two-year college or technical school; or six months to one year related experience and / or training or equivalent combination of education and experience.
  • Must be familiar with local resources for homeless youth.
  • Must be at least 21 years old.
  • Be able to complete specialized training including First Aid, CPR, and Medication Certification.
  • Knowledge of and the ability to interpret and apply laws, rules, policies, and procedures relating to housing assistance programs.
  • Adapts to changing business needs, conditions, and work responsibilities; responds to change with a positive attitude and a willingness to learn new ways to accomplish work activities and objectives.
  • Knowledge and demonstrated ability to provide clinically guided supervision and services related to mental illness, physical disability, substance use, and domestic violence.
  • Demonstrates responsible personal and professional conduct; accepts personal responsibility for the quality and timeliness of work.
  • Applies effective written and verbal communication techniques to convey clear and timely expectations, listens to people's ideas and concerns, resolves confrontations, disagreements, and complaints in a constructive manner.
  • Ability to exercise independent judgment, using analysis, experience, and perspective in solving problems or communicating decisions or actions.
  • Effective interpersonal skills, with the ability to work collaboratively to garner trust, respect, and confidence from others.
  • Ability to work effectively under pressure in a fast-paced environment.
  • A collaborative team member who is also a strong leader with well-developed organizational and time management skills.
  • Ability to develop effective working relationships; works cooperatively, exchanges ideas, and addresses issues in a constructive manner.
  • MS Office experience, especially Outlook.
  • Must pass a criminal records check and pre-employment drug screening.
  • Must have a valid driver's license and current automobile insurance.
  • Hours needed :

    Business hours, Monday-Friday with weekly on-call rotation
