Job Title : Advanced Practice Clinician- Nurse Practitioner / Physician AssistantLocation : Long Branch, NJ Monmouth and Ocean CountiesContract : 12 Months (Possibility of Extension)Schedule : Mon-Fri, 8 : 00 AM - 5 : 00 PMPay Range : $65 - $70 / HourResponsibilities : Conduct patient assessments.Manage acute and chronic conditions.Collaborate with the care team.Handle advanced care planning.Offer patient and caregiver education.Requirements : Active state licenseNP : Master’s or Doctoral degree in Nursing.PA : Accredited Physician Assistant program.Board certification (NP or PA).Current DEA licensure.Ability to meet physical job requirements.Reliable transportation.Full COVID-19 vaccination (proof required).Preferred : 1+ year of clinical experience (long-term care setting preferred)Knowledge of geriatrics and chronic illness.
