Job Title : Nurse Practitioner - Primary Care and Wound Care in Transitional and Long-Term Care Units, Assisted Living Facilities
Job Summary : We are seeking a dedicated and experienced Nurse Practitioner to provide comprehensive primary care and specialized wound care services in Transitional Care Units (TCUs) and Long-Term Care Units (LTCs), as well as Assisted Living Facilities. The ideal candidate will play a vital role in ensuring the well-being and recovery of patients by delivering high-quality care, managing complex wound cases, and collaborating with interdisciplinary healthcare teams.
Key Responsibilities :
Primary Care Responsibilities :
- Patient Assessment : Conduct comprehensive assessments to establish individualized care plans and treatment goals for residents / patients in TCUs and LTCs.
- Care Management : Diagnose, treat, and manage chronic and acute medical conditions, ensuring patients receive appropriate and timely care.
- Medication Management : Prescribe, manage, and monitor medications, providing education on proper usage and potential side effects.
- Preventive Care : Deliver preventive care services and health education to residents / patients and their families, focusing on maintaining long-term health.
- Documentation : Maintain accurate and up-to-date medical records, adhering to all regulatory requirements and facility policies.
- Emergency Response : Address medical emergencies promptly, coordinating with the healthcare team and ensuring patient safety.
Wound Care Responsibilities :
Wound Assessment and Diagnosis : Perform detailed wound evaluations, including identifying wound types, stages, and underlying causes.Treatment Planning : Develop and implement individualized wound care treatment plans, utilizing advanced wound care techniques and evidence-based practices.Specialized Interventions : Provide specialized wound care treatments, such as debridement, application of advanced dressings, and use of wound healing technologies.Infection Control : Monitor wounds for signs of infection and implement appropriate interventions to prevent complications.Patient and Staff Education : Educate patients, families, and facility staff on wound prevention, care techniques, and management of pressure ulcers and other chronic wounds.Collaboration : Work closely with interdisciplinary teams, including dietitians and physical therapists, to optimize wound healing and overall patient outcomes.Quality Improvement : Participate in quality improvement initiatives related to wound care, contributing to best practices and improved patient care processes.Documentation : Accurately document wound assessments, treatments, progress, and outcomes, ensuring compliance with regulatory and clinical standards.Qualifications :
Master's or Doctoral degree in Nursing (NP)Current state Nurse Practitioner licenseWound Care Certification (WCC, CWOCN, or equivalent) or willingness to obtainCertification as a Nurse Practitioner in Adult-Gerontology, Family, or another relevant specialtyExperience in primary care and wound care, including managing chronic and complex woundsStrong clinical, analytical, and assessment skillsExcellent communication and interpersonal skillsProficiency in electronic health records (EHR) systemsCompassion and dedication to delivering patient-centered careWillingness to train in these areas if you do not have the qualifications is an opportunity as wellWorking Conditions :
