Under the Direction of the Assistant Director for Clinical Development and Medical Director of Cardiac Services, the Cardiology LPN supports patient care, throughput and clinical operations for the Heart Center, Cardiology Resident Clinic and Benning Road Cardiology site.
Specific Duties Are as Follows :
Trains all staff on department pre-visit planning protocolsConducts telephone triage, as scripted per the red / yellow / green triage matrixReviews referrals and schedules based on diagnosis protocolsReconciles orders, including EKGs, Echos, X-rays and Stress TestsCompiles consult notes and sends to referring providersLiaison for inpatient discharge and ER follow-up appointmentsTracks patients for value-based payments programsTrains medical assistantsManages Cardiology task in-basket, ensures service levels are metManages patient throughput for Department Clinical TrialsCollaborates with Clinic Supervisor and Clinical Director for improvement projectsCollaborates with physician / medical residents as appropriate to determine action plan / escalation for cardiology patientsDocuments telephone triage call in the EMR.Manages Medication Refills processReviews patient EMR for prescribed medication indication / historyConfirms medication prescription to ensure it is the right medication, dosage, and frequency reviews allergies / changes in medical historyContacts patient to schedule appointment if required for refillPends refill request to physician in EMRDocuments / scans all relevant information in EMR.Other duties as assignedEducation
Licensed Practical Nurse - RequiredDC License - Required2+ years experience in an academic health system - Preferred3+ years ambulatory nursing experience or 2 years inpatient experience on a med / surg Unit - RequiredCompliance Salary Range Disclosure
