Program Description : The Assertive Community Treatment (ACT) Team is a unique and nontraditional licensed outpatient psychiatric clinic for individuals who have demonstrated a high service need with multiple hospitalizations, emergency room visits and difficulty maintaining stability in the community. ACT is a nationally recognized model and an evidence-based practice. The ACT Team serves the Upper West Side, Harlem, Washington Heights and Inwood.

Purpose of Position : The Psychiatric Nurse Practitioner is responsible for delivering direct and comprehensive psychiatric care for the ACT team clients which includes prescribing medications and completing psychiatric evaluations.

Schedule : 14 hours / week. Flexible.

Hourly Rate : $115.00 - $135.00

Roles, Responsibilities, and Essential Duties

1) Psychiatric

• Provide psychiatric treatment to 34 clients
• Conduct monthly psychiatric sessions with each client
• Prescribe medication when appropriate
• Complete psychiatric evaluations as needed
• Assess psychiatric and medical status of clients
• Complete required paperwork following program guidelines
• Communicate with clients' families about their needs and progress
• Serve as liaison to other service providers
• Visit clients in residential facilities, on the street and in other locations
• Be available by telephone after hours an on weekends to assist the team with urgent client interventions
