Responsibilities

• The Hospice RN provides nursing care and coordinates care for terminally ill patients of all ages
• Serves in a supervisory capacity by directing and evaluating paraprofessional staff. P
• Plans, provides, evaluates, coordinates care and provides case management and documents in a manner which meets the quality, efficiency, and care standards identified by Agency Policies and Procedures and according to professional standards of home health nursing practice.
• Acts as case manager and is responsible for overseeing the ongoing care planning management, coordination of care, communications and for outcomes of the plan of care.
• Acts to uphold the Agency’s mission, clinical expertise and maintenance of standards of care for Hospice and Palliative care and fiscal soundness within a changing health care environment.

Our interdisciplinary hospice team provides a personalized plan of care focused on pain and symptom management, while emphasizing quality of life and comfort.
