GENERAL SUMMARY :

Develops a health coaching relationship with wellbeing program participants and assists them through the process of actively working towards better health by providing support, encouragement, and education. Coordinates the implementation of health driven programs to provide health improvement measurements and results to affect the overall health of Bryan Health wellbeing program participants. Programming will include, but not be limited to, nutrition and health education, health screenings and health fairs, health communications and health coaching, awareness events and classes, lifestyle interventions and high-risk management, and chronic disease prevention and management. Works in a team-based environment collaborating with internal and external resources to provide support for clinical issues.

PRINCIPAL JOB FUNCTIONS :

  • Commits to the mission, vision, beliefs and consistently demonstrates our core values.
  • Actively engages participants and facilitates the implementation of their customized care plans.
  • Facilitates the creation of an agenda and works with the participants to ensure a pragmatic care plan is established.
  • Implements a range of health-centered skills including : capturing relevant information about the participant’s health and healthcare experience, taking vitals, collecting measurements, administering point-of-care testing, and performing standard screening assessments.
  • Identifies health concerns of participants and applies appropriate frameworks, principles, and models to solve or manage the problem.
  • Takes responsibility for the health of a population of participants and monitors and tracks their chronic health management and / or preventive measures.
  • Understands the chronic health conditions that spans the participant population.
  • Provides participant education on chronic health management and prevention.
  • Establishes meaningful, health-centered relationships with participants.
  • Employs confidence-promoting techniques in program communication and develops self-efficacy to better manage health.
  • Coaches participants using an action-planning model based on motivational interviewing techniques.
  • Communicates with participants by phone and email and collaborates / partners with clinical staff, when necessary.
  • Provides individualized health coaching to participants in obtaining their health objectives.
  • Creates and adapts wellness objectives to overcome barriers to good health and wellness based on needs of, and with input from, participants.
  • Co-facilitates groups / classes to support improved health outcomes.
  • Helps participants understand their chronic conditions, why they matter, and how they can manage them.

17. Collaborates with other team members to facilitate group events and classes for participants.

18. Coordinates with Health Coach to expand the reach of wellness / health focused services.

  • Provides administrative support to assist with the coordination of care across the continuum, such as : scheduling appointments with specialists, coordinating referrals, sharing or transferring information with the participant’s internal and external care team.
  • Promotes involvement and engagement of chronic health management and prevention programs with primary care.
  • Understands core insurance benefits to ensure participants take full advantage of them and are well-connected with benefits specialists.
  • 22. Presents on areas of interest / expertise in health and wellness to participants and community members

  • Completes all tracking and reporting requirements for outcomes and evaluation.
  • 24. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

    25. Participates in meetings, committees and department projects as assigned.

    26. Performs other related projects and duties as assigned.

  • REQUIRED KNOWLEDGE, SKILLS AND ABILITIES :

    1. Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient’s growth and developmental needs, annual skill competency verification and mandatory education and competencies.

    2. Knowledge of computer hardware equipment and software applications relevant to work functions.

    3. Ability to communicate effectively both verbally and in writing.

    4. Ability to perform crucial conversations with desired outcomes.

    5. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff.

    6. Ability to problem solve and engage independent critical thinking skills.

    7. Ability to provide quantitative program reporting and analysis.

    8. Ability to maintain confidentiality relevant to sensitive information.

    9. Ability to prioritize work demands and work with minimal supervision.

    10. Ability to maintain regular and punctual attendance.

    EDUCATION AND EXPERIENCE :

    Current license as Licensed Practical Nurse (LPN), Registered Nurse (RN), Registered Dietitian (RD), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or two (2) years of relevant chronic care management and / or health coaching experience.

    Experience in disease management / health coaching program with health plan or similar organization strongly preferred. Training in motivational interviewing is preferred. Previous experience / education with asthma, diabetes, cardiac conditions, weight-related conditions, musculoskeletal disorders, maternity, and / or depression education programs preferred.

    OTHER CREDENTIALS / CERTIFICATIONS :

    Basic Life Support (CPR) certification required within 90 days of hire date. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
