Job Summary :
We are seeking a skilled Registered Physical Therapist (PT) to join our team and provide expert physical therapy services to patients in their homes. You will work under the direction and supervision of the Director of Clinical Services / Clinical Manager and provide patient care in accordance with physician orders and individualized care plans. You will play a critical role in helping patients improve their physical function and return to a productive life.
Responsibilities :
- Adhere to established policies and procedures in providing physical therapy services.
- Provide physician-prescribed physical therapy to improve or minimize the patient’s physical disabilities.
- Return patients to the highest possible level of functionality and independence.
- Collaborate with other home care team members in developing and executing patient care plans.
- Direct and supervise assigned personnel, including Physical Therapy Assistants and Home Health Aides.
- Perform initial assessments, develop the plan of care, implement maintenance programs, and conduct reassessments every 30 days.
- Carry out all skilled physical therapy procedures as ordered by the physician.
- Consult with physicians regarding changes in the patient’s treatment or progress.
- Provide written reports to physicians on the patient’s progress and response to therapy.
- Educate patients and their families or significant others on home programs and activities of daily living.
- Participate in inservice programs and present inservice sessions as required.
- Contribute to Quality Assessment and Performance Improvement (QAPI) initiatives.
- Attend all scheduled patient care conferences.
- Prepare and maintain accurate clinical and progress notes.
- Assist physicians in evaluating patients' levels of function and revising care plans as necessary.
- Consult with family members and Agency personnel on patient care and progress.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition as per Agency-defined time frames.
- Appropriately apply ICD-10 coding for patient diagnoses.
Working Environment :
Works indoors in the Agency office and patient homes. Travel to / from patient homes is required.Job Relationships :

Ability to participate in physical activity, including heavy lifting, bending, and standing for extended periods.Ability to assist with patient mobility and provide therapeutic exercises.Requirements : Qualifications :
Graduate from a physical therapist curriculum accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), the American Physical Therapy Association (APTA), or an equivalent body.Current state licensure in the field of physical therapy.Two (2) years of experience preferred.Acceptance of the philosophy and goals of the Agency.Ability to exercise initiative and independent judgment.
