Collaborate with the leadership team in developing and implementing innovative additional care strategies.
Oversees and conducts primary care screenings and monitoring of key health indicators and risks for clients per CCBHC requirements. This includes assessing the need for further physical exams or evaluations by a primary care provider.
Ensure timely and accurate screening for all clients.
Oversees the coordination and facilitation of referrals for off-site laboratory testing, ensuring timely submission and retrieval of results to support integrated care and effective treatment planning.
Hire, train, and provide supervision, guidance, and support to Medical Assistants and Client Navigators in all aspects of their roles, fostering professional development and ensuring quality performance.
Exceptional interpersonal and communication skills.
Provide health education to clients.
Demonstrated ability to collaborate with clients experiencing mental health and / or substance use challenges.
Strong knowledge of primary care and behavioral health integration.
Strong proficiency in computer skills, including the ability to work with medical software.
Ability to manage multiple changing priorities in an effective and organized fashion.
Trained in administering Naloxone.
Requirements
Active & Clean license as an Advance Practice Registered Nurse (APRN) with certification as a Certified Nurse Practitioner (CNP) in the state of New Mexico.
At least two (2) years of clinical experience; prior experience in a behavioral health or integrated care settings is strongly preferred.
Previous supervisory or leadership experience is preferred, demonstrating the ability to manage and support a team effectively.
