Description

Summary :

Provides Speech Pathology Services to patients referred,including assessment, treatment plan development and implementationwith follow-up and discharge planning. Assists with clinicalaspects of the department, including program development andPerformance Improvement activities. In addition, supervises RehabTechs, Students and Volunteers.

Responsibilities :

  • Performs comprehensive diagnostic evaluationsto determine the status of the speech, language, cognitive andswallowing abilities of inpatients and outpatients, consideringdiagnosis and age specificneeds.
  • Develops and implements, withpatient / family participation, individualized treatment plans asindicated, considering diagnosis and age specific needs, withmeasurable goals and methods to achieve goals.
  • Maintains quality documentation regarding patient status, toinclude : Initial Patient Evaluations, Daily and Weekly Progressnotes as required, Discharge Summaries and other notations asappropriate.
  • Participates in patient careconferences, family conferences, in-service programs, departmentaland interdepartmental meetings.
  • Counselsfamily and other involved individuals regarding patient evaluation,care and treatment. Initiates and completes discharge planning in atimely manner and provides effective education andtraining.
  • Supervises Speech Pathologystudents and volunteers.
  • Assists in thesupervision of Rehab Techs.
  • Participatesin the orientation of new associates and scheduling patients toassure quality patient coverage.
  • Understands,supports and provides input for Performance Improvementactivities.
  • Complies with departmentalproductivity standards.
  • Maintains an awarenessof other agencies within the community in order to make appropriatereferrals, when indicated, to network and to increase knowledge ofcommunity resources.
  • Identifies and provideseducational opportunities for self, department, students andcommunity. Participates in research activities asappropriate.
  • Demonstrates competence toperform assigned patient care responsibilities in a manner thatmeets the population-specific and developmental needs of patientsserved by the department.
  • Appropriately adaptsassigned patient care assessment, treatment, and / or care methods toaccommodate the unique physical, psychosocial, cultural, agespecific and other developmental needs of each patientserved.
  • Demonstrates adherence to the COREvalues of Santa Rosa Health Care.
  • Performsother duties as assigned.

Requirements : EDUCATION :

  • Master’s degree from an accredited college oruniversity in the area of Speech Pathology isrequired.
  • EXPERIENCE :

  • A minimum of one and one-half (1.5) yearsfull-time clinical experience as a SpeechPathologist.
  • LICENSURE / CERTIFICATION :

  • Licensed as a Speech Pathologist by the StateBoard of Examiners for Speech-Language Pathology and Audiology.
  • Certified by the AmericanSpeech-Language-Hearing Association.
  • CPRcertification is required.
