KEY POSITION SPECIFIC ROLES & RESPONSIBILTIES
- Formulates and coordinates interdisciplinary discharge plans for residents. Assures successful transfer
of residents both within the center / campus and to outside agencies. Assures leave of absences are
processed smoothly. Assures all aspects of admissions process of residents is done in compliance with
state and federal regulations along with FCC Guidelines.
Performs clinical evaluations on assigned residents.Assesses and develops a plan of care that meets residents' needs. Provides or arranges for socialservices for residents / family residents as appropriate.
Safeguards residents' property. Maintains residents' self-esteem, privacy and confidentiality ofpersonal information. Provides residents with opportunities for independence and choice consistent
with their care plans. Assures residents' living areas are pleasant, orderly and personalized.
Completes all documents related to social work. Receives and shares information; observes and reportsresidents' conditions to appropriate staff. Educates families and residents regarding placement, care
and rights. Teaches staff on issues related to resident care, rights and behaviors.
Formulates and works within annual budget. Accepts accountability for activities under direct control;plans and organizes work with and for department. Exercises sound judgment and decision-making;promotes cooperative working relationships within and between departments. Acquires resources and
means that promote a quality work environment. Assists and participates in the QA / CQI process
ensuring quality performance, conformance with regulations, standards and laws, and customer
satisfaction. Keeps abreast of and implements new programs or practices.
Interviews residents and / or family prior to admission, ensuring that all appropriate admission formsare completed.
Collaborates with external psychologists, psychiatrists, and / or providers to ensure theirrecommendations are integrated into the resident's plan of care in an effort to support psychosocial
needs.
Promote and monitor the implementation of the Resident Bill of Rights as well as functioning as theResident's advocate.
Tours facility with resident and / or family, providing information on admission policies, procedures,and other information of concern to families and at family request.
Meets emotional needs of resident and / or family by assisting with acceptance of losses, changes infamily relationships and financial status.
Ensures that Social Service intervention is recorded on required documents and that appropriate actionis taken.
Assist residents with problem solving and completing a resident satisfaction survey form asappropriate in keeping with facility policy.
Reviews resident's chart and contacts family to gather background material in order to develop a socialhistory on new admissions.
Serves as liaison between residents, families, the facility and the surrounding community.Assists with discharging planning for residents who are transferring or returning to the community.Provide information on advance directives and engage in discussion as necessary.Assist residents and families with financial and business matters in conjunction with the BusinessOffice and Admissions department.
Attends initial staffing and care plan conferences, assisting in the development of goals for residents.Maintains records of all Social Worker contacts with family, friends or community agencies on behalf ofresidents.
Participates in Social Service policy development, orientation and in-service training of personnel.Participate in continuous quality improvement (QAPI) efforts; serve on relevant task forces, projectteams and committees.
Organizes and facilitates family support groups at regular intervals.Facilitates psychosocial groups and / or one on ones as needed.
