PharmD Live is a rapidly growing healthcare startup dedicated to delivering high-quality patient care through board-certified clinical pharmacists. We are seeking a dynamic On-Site Patient Enrollment Specialist (PES) to join our team. This role involves enrolling patients in the Chronic Care Management (CCM) program at clinic locations. The PES will primarily engage with patients face-to-face, with minimal telephonic enrollments, to communicate the value of CCM and other clinical programs.

The ideal candidate is highly self-motivated, an excellent communicator, and thrives in a fast-paced, patient-centered environment. This is an on-site role requiring strong interpersonal skills and a results-oriented mindset.

Key Responsibilities :

  • Engage with patients in clinics to explain the benefits of PharmD Lives Chronic Care Management (CCM) program and other services.
  • Enroll patients directly into the CCM program during face-to-face interactions at clinic locations.
  • Rotate between high-traffic clinics to maximize patient outreach and enrollments.
  • Schedule patient appointments with clinical pharmacists as part of the enrollment process.
  • Respond to patient inquiries and provide clear, empathetic communication to address concerns.
  • Collaborate with clinic staff and physicians to streamline the enrollment process and enhance patient engagement.
  • Document enrollment activity and patient interactions accurately in company systems.
  • Assist with patient eligibility verification and related administrative tasks as needed.
  • Provide occasional telephonic support for patient enrollment, if required.
  • Participate in team meetings and provide feedback to improve service delivery and program efficiency.

Requirements

  • High school diploma or equivalent required; college coursework in healthcare or related fields is a plus.
  • 3+ year of medical clinic experience with a proven track record of success.
  • Prior experience in a healthcare setting or medical sales preferred.
  • Strong interpersonal skills with the ability to build trust and rapport quickly with patients and clinic staff.
  • Exceptional organizational skills and attention to detail.
  • Ability to thrive in a high-energy, goal-oriented environment.
  • Flexibility to travel between assigned clinics and adapt to varying schedules.
