A company is looking for a Clinical Consultation Pharmacist.Key ResponsibilitiesConduct telephonic consultations with patients regarding specialty medication and disease state managementPerform medication reconciliation, screen for drug interactions, and provide side effect management recommendationsInterpret prescriptions, contact prescribers for clarifications, and ensure compliance with internal and external regulationsRequired QualificationsGraduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degreeCurrent, unrestricted pharmacist license in state of residenceAt least two years of experience as a practicing licensed Pharmacist preferredStrong clinical background requiredExperience and / or understanding of Specialty Pharmacy medications desirable
