Position : Full-Time Licensed Practical Nurse (LPN)

Location : Phoenix, AZ (Territory Coverage)

Compensation and Benefits :

  • Competitive pay per visit starting at $41 / visit with a $30 / NVA

Qualifications :

  • Graduate of an accredited practical nurse or vocational nursing program
  • Minimum one (1) year of experience, preferably in home care or med / surg
  • Current licensure in the state and CPR / BLS certification
  • Strong communication and problem-solving skills
  • Ability to meet physical demands of the job
  • Valid driver's license and reliable, insured vehicle
  • Responsibilities :

  • Implement personalized care plans under clinician supervision
  • Maintain accurate and timely documentation
  • Provide patient and family education on diagnoses and care plans
  • Coordinate with interdisciplinary team members for comprehensive care
