The Utilization Manager will clinically supervise assigned case managers across various sites, ensuring clients' length of stay is monitored, and target move-out dates are established. The role involves supporting case managers in the development of comprehensive housing and care plans, ensuring clients are progressing toward successful transitions, working to improve client outcomes, reduce shelter recidivism, and align with the necessary services. This position coordinates with internal and external stakeholders such as site manager and referral specialist to ensure clients receive appropriate care. The Utilization Manager provides specialized case management services for CCS clients that include supportive counseling, service coordination, and crisis intervention.

Compensation :

Starting from $60,000 annually, actual pay commensurate with experience.

Essential Functions :

• Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
• Review and manage the use of our specialty / medical house beds, ensuring that all necessary information is gathered and processed; Work collaboratively with housing navigators and other external stakeholders to meet the needs of Level 1 and 2 clients.
• Facilitate the communication between shelter case managers, housing navigators, and other team members for coordinated care and introduce new shelter clients to services and provide initial support and orientation.
• Support case managers & referral specialists in developing short-term and long-term goals aligned with clients' housing and care plans; Assist in creating detailed housing and care plans for new clients, working closely with the case managers to ensure comprehensive support.
• Review and manage client referrals to the CIS program, ensuring all required documentation is completed and submitted; Maintain and update the CIS tracker with current client statuses and ensure timely follow-ups.

Regular full time classification. Generally Monday through Friday and some weekends. Hours and days may vary based on the needs of the consumers and the organization.
