As a LPN Quality Reviewer, you will play a pivotal role in coordinating and managing patient care to ensure the highest standards are met. This position involves supervising clinical personnel and ensuring the delivery of quality home care services.
RESPONSIBILITIES :
1. Receive case referrals and assess patient needs to assign appropriate clinicians.
2. Review and evaluate each case, providing guidance to clinicians for effective performance.
3. Reviews and appropriately codes all Oasis Assessments
4. Instruct and guide clinicians to promote quality care delivery, being available to assist as needed.
5. Review patient clinical information, including diagnosis, medications and procedures.
6. Assist in establishing therapeutic goals and developing care plans.
7. Organize and lead case conference meetings to facilitate care coordination.
8. Conduct quarterly record reviews and communicates findings to appropriate personnel.
9. Assist screening, interviewing, and orienting new personnel.
10. Assist in planning and implementing in-service and continuing education programs.
11. Contribute to the formulation, revision and implementation of policies and procedures.
12. On-Call Rotations for after-hours services.
13. Perform direct patient care duties as needed.
14. Maintain compliance with professional standards and principles.
15. Manage a team of Clinicians and follow up to ensure all necessary documentation is completed timely and follow up actions as indicated.
QUALIFICATIONS :
- Licensed Practical Nurse (LPN) with current license in Virginia
- Bachelor’s degree in nursing or related field, preferred
- Coding Certified ICD-10, preferred
- Previous experience in home care or healthcare management, preferred
- Strong leadership and communication skills
- Ability to prioritize and multi-task effectively
- Proficient in Microsoft Office Excel, Microsoft Word and electronic medical records systems
QUALIFICATIONS :
Pay : $24.00 hr up to $26.00 hour
