Shift : DayWorking Hours : Monday-Friday 8 am-5 pmSummary : The PHSO Patient Outreach Specialist plays a key role in improving patient outcomes by addressing health care gaps and ensuring compliance with quality metrics. This position is responsible for requesting, retrieving, and uploading patient records, facilitating patient scheduling, and working with electronic health records (EHR) and payor portals to close health care gaps through record submissions and reconsideration processes. The ideal candidate is organized, detail-oriented, and committed to enhancing patient care and quality improvement efforts.Other information : High school diploma or equivalent required; associate's degree or higher in healthcare- related field preferred. Experience in clinical support roles, such as medical assistant, healthcare administrative assistant, or similar, preferred. Strong organizational skills and attention to detail, especially when managing clinical orders and ensuring timely provider action. Excellent communication and interpersonal skills to effectively coordinate with providers, staff, and patients. Familiarity with healthcare workflows, including order placement and provider approval processes. Ability to work independently and as part of a team to manage priorities and meet deadlines.Preferred Skills : Proficiency in using electronic health record (EHR) systems to manage and track pending orders. Knowledge of medical terminology and healthcare documentation practices. Experience in patient outreach or customer service roles. This job will be authorized 80.00 hours bi-weekly.
