POSITION : Nurse Coordinator

POSITION PURPOSE :

Provide direction, coordination, supervision and implementation of professional and supportive services to Home Care Agency (HCA) consumers. Monitor services provided to HCA consumers, provide training to attendants and ensure consumer satisfaction and safety. Complete and submit all required documentation.

Pay Range : $36 - $42 / hour DOE + $5,000 sign-on bonus

Location : Primary caseloads in Westminster, Broomfield, and Lakewood with outliers in Longmont, Loveland, and Boulder. OR Primary caseload in Aurora, Centennial, and Littleton.

Work Environment : Hybrid; Remote and in the field with Tu / Th in office
