Communication with team and clients Medical documentation Annual In-Person Supervisory Visits Perform record audits Education to clients, employees and families Utilizing Carevoyant software program 1 year experience practicing as an RN (required) with additional experience in acute care Med / surg experience highly preferred Working knowledge of disabled community is preferred. A person with a "disability" is someone who has a physical, mental or communication disability that substantially limits one or more of the major life activities such as caring for oneself, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning and working. 1 year of supervision experience overseeing caregivers and other care providers Spanish speaking (highly preferred) Current, unencumbered state certification to practice as an RN in the state of Colorado BSN highly preferred Current CPR / First Aid certification

PASCO is seeking a dedicated HCA Nurse Coordinator to join our team! As an HCA Nurse Coordinator, you will play a crucial role in coordinating and overseeing healthcare services for our clients located in Lakewood, Littleton, and Denver metropolitan area. If you are a compassionate and organized individual with a passion for healthcare, we would love to have you on our team! - Coordinate and manage healthcare services for clients
- Collaborate with healthcare providers to ensure quality care
- Assess client needs and develop care plans
- Provide guidance and support to healthcare staff

POSITION : Nurse Coordinator
POSITION PURPOSE :
Provide direction, coordination, supervision and implementation of professional and supportive services to Home Care Agency (HCA) consumers. Monitor services provided to HCA consumers, provide training to attendants and ensure consumer satisfaction and safety. Complete and submit all required documentation.
