Job descriptionDeliver patient care and assess physical, psychosocial, and environmental needs under RN supervision. Review the Plan of Care prior to patient visits and deliver care according to established guidelines while documenting appropriately. Conduct ongoing assessments during visits, documenting significant findings and changes to the patient’s condition for communication with the RN and physician. Evaluate patients' knowledge regarding their health conditions and treatment plans, providing tailored education and recognizing additional healthcare needs. Basic Cardiac Life Support certification is required within 30 days of employment. Current Licensed Practical Nurse (LPN) certification. Valid Driver's License. Graduate of an accredited Vocational School.

Role Summary
As a Licensed Practical Nurse in home health, you will provide high-quality nursing care to patients in their homes. Working under the direction of a Registered Nurse (RN), you will follow physician orders as per the Plan of Care.
