We are seeking a Permanent Social Worker to join our dynamic healthcare team. This position involves offering psychosocial support to patients and their families, particularly those dealing with terminal illnesses and end-of-life care. The ideal candidate has a strong background in psychiatry and is adept at conducting comprehensive assessments. This role requires a minimum of 5 years of experience in a similar capacity, with a focus on empathetic, patient-centered care.Responsibilities : As a Permanent Social Worker, you will be responsible for : 1. Providing emotional and psychosocial support to patients and their families, particularly those dealing with terminal illness and end-of-life care situations.2. Conducting comprehensive psychosocial assessments and developing individualized care plans based on these assessments.3. Collaborating with a multidisciplinary team of healthcare professionals to ensure the holistic care of patients.4. Navigating complex medical information and explaining it in a way that patients and their families can understand.5. Advocating for patients' rights and needs, ensuring they are being met throughout their medical journey.6. Providing crisis intervention as needed and facilitating access to appropriate mental health resources.7. Assisting patients and their families in understanding and making decisions regarding treatment options, advance directives, and other end-of-life decisions.8. Coordinating with community resources to ensure continuity of care post-discharge.Qualifications : The ideal candidate for the Permanent Social Worker position will possess the following qualifications : 1. A Master’s degree in Social Work from an accredited institution.2. A minimum of 5 years of experience as a social worker, preferably in a medical or psychiatric setting.3.
