ChildNet, Inc. is a private, not-for-profit community-based care (CBC) agency servicing Broward and Palm Beach County. We are contracted with the State of Florida, Department of Children and Families, to provide case management support to abused, abandoned, and neglected children. ChildNet is nationally accredited by the Council on Accreditation (COA) and employs over 450 staff at its location in Ft Lauderdale and West Palm Beach, Florida.
Job Summary : This position is responsible for reviewing and collecting information on cases referred to case management by the Department of Children and Families. Collaborates with internal and external stakeholders to measure outcomes and meet service and quality standards.
The items listed below are intended to provide an overview of the essential functions of the job. This is not an exhaustive list of all functions and responsibilities that the position may be required to provide.
- Complete Dependency Case Management job duties.
- Collect data according to contract requirements and provide reports as needed.
- Participate in local review committee meetings.
- Meet with the Dependency Drug Court child(ren) and family in their home.
- Gather information about the child(ren) and family including the make-up of the household, social history, previous abuse reports, educational, medical, and behavioral health history, and circumstances surrounding alleged maltreatment.
- Conduct face to face home visits with the Dependency Drug Court parents.
- Proactively develop, assess, and maintain up-to-date case plans to accurately reflect the Dependency Drug Court child(ren) and families' progress over time. Promote family involvement in services to include case reviews and staffings.
- Evaluate, prepare reports, make recommendations, and decisions on the Dependency Drug Court child(ren) and families.
- Attend all court hearings and make recommendations to the judge on behalf of the child(ren) as sees fit. Assists the Dependency Drug Court child(ren) in understanding all court proceedings.
- Interact with Dependency Drug Court families to establish needs and required services for clients through referrals for services. Works with clients to provide and refer services to meet needs. Provide support to foster homes and relative placements.
- Coordinate monthly face-to-face visits with Dependency Drug Court child(ren) and family in the home. Observes reports and makes recommendations based on observations. Provides Dependency Drug Court child(ren) with opportunities to visit immediate family, relatives, and others with court ordered visitation.
- Perform other duties as assigned.
