What it's like to be a Case Manager-Reinvestment at Case Manager-Reinvestment
As a full-time case manager at the Hampton-Newport News Community Services Board, you will play a vital role in the Regional Reinvestment Project to optimize mental health care by reducing reliance on state psychiatric facilities. Your responsibilities will include monitoring inpatient treatment and crafting effective discharge plans that facilitate seamless transitions to community support systems. You will provide short-term case management, typically lasting less than ninety days, ensuring that individuals receive the necessary resources and services for their ongoing recovery.

What we're looking for in a Case Manager-Reinvestment
To excel as a full-time case manager at the Hampton-Newport News Community Services Board, a solid foundation in Human Services is essential. Candidates should possess a Bachelor's degree in fields such as Social Work, Psychology, Sociology, or Counseling, paired with a minimum of one year of experience in case management services. Proficiency in relevant software and tools for case tracking and documentation is crucial for effectively managing client cases.
The ability to establish rapport with clients and navigate community resources is paramount for successful discharge planning and ongoing support. Additionally, a valid Driver's License is required, as transporting clients may be necessary to facilitate access to essential services.
