JOB SUMMARY : The Clinical Pharmacy Specialist provides pharmaceutical care services through direct interaction with patients and providers, at the highest level of clinical practice, working in close collaboration with other clinical team members and serves as a consultant for intensive medication therapy management services. The Clinical Pharmacy Specialist may also serve as pharmacy representative to health center committees as requested; assist in student preceptor programs; participate in research programs where appropriate; and provides appropriate leadership to other pharmacy staff. The Clinical Pharmacy Specialist reports directly to the Pharmacy Clinical Coordinator.
MAJOR DUTIES AND RESPONSIBILITIES : Provision of safe, effective, appropriate, and cost-effective medication therapy to patients in area of psychiatric ambulatory care. Under direction of the Clinical Pharmacy Director and Chief Medical Officer, the Clinical Pharmacy Specialist will assist in the planning, organizing, and monitoring of the delivery of psychiatric ambulatory care pharmacy services for the Chase Brexton Health Care Centers. These responsibilities include, but are not limited to, the following : Clinical Service Provision
- Prescribe medications under a collaborative practice agreement with psychiatrists and mid-level psychiatric providers
- Offer psychiatric pharmacotherapy consultations
- Guide treatment decisions and assist with dosage adjustments
- Screen for drug interactions and manage adverse reactions
- Provide medication education to patients and staff members
- Assist with transitions of care / polypharmacy review / comprehensive medication management (CMM)
- Manage long-acting injectable (LAI) antipsychotics and clozapine therapy Performance Management
- Supervises clinical pharmacy technician and delivers performance evaluations
- Provides appropriate coaching and counseling to clinical pharmacy technician

Provide services such as, but not limited to, monitoring patients in specialty services, collecting patient-specific data, interpreting drug related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.

SKILLS AND ABILITIES :
EDUCATION AND / OR EXPERIENCE :
Two (2) years of recent psychiatric pharmacy experience as a clinical pharmacist and / or experience in internal medicine in either the inpatient or outpatient setting providing clinical service to psychiatric patients.
