Are you a compassionate and experienced Case Manager looking for a rewarding opportunity in Phoenix, AZ? We are a third-party recruitment agency seeking a dedicated and skilled Case Manager to join a respected organization. This role offers competitive pay and the chance to make a meaningful impact in the community.
Key Responsibilities :
- Assess and evaluate clients’ needs to develop individualized care plans.
- Coordinate and monitor services for clients, ensuring they receive appropriate support.
- Advocate for clients and connect them with community resources and services.
- Maintain accurate and up-to-date case records and documentation.
- Collaborate with healthcare providers, social workers, and other professionals.
- Provide ongoing support and follow-up to ensure clients’ needs are met.
Qualifications :
Pay Details : $22.00 to $25.00 per hour
