Description
This position involves providing direct services to individuals. This program is funded to provide care for patients / clients who do not often fit well within the standard medical model. This position will work closely with medical providers, primary care teams, and other agencies to improve patient outcomes and care. The Doula Community Health Worker (CHW) in the STAR (Sustaining Treatment and Recovery) Moms Program supports pregnant and postpartum individuals at risk for substance use disorder and their families. This role provides comprehensive social, emotional, and medical support, focusing on recovery, risk reduction, and building resilience. The Doula CHW facilitates care coordination between healthcare providers, assists with social service navigation, and supports perinatal health, ensuring that participants receive the necessary support to achieve positive health outcomes. This position will work closely with their health center site, their PCPs, Local Health Departments, and Community Based Organizations to provide wrap-around services and create a foundation of trust for continued care of socially vulnerable populations.
Examples of Duties
Support - Direct Patient Interaction 100%
1. Explain program requirements and guidelines to the general public.
2. Develop community resource materials that are up to date and reflective of the local community.
3. Conduct comprehensive interviews and intake assessments to determine the needs of individuals to be served by the program.
4. Assist patients in setting and achieving personal goals in person, by phone or in group setting.
5. Through skill building interventions and other supportive interventions, assist patients in effectively utilizing a wide spectrum of resources, managing symptoms of their illness, and developing daily living skills to gain greater independence, employment and other meaningful activities.
6. Connect patients to community resources including but not limited to housing, medical care, education, vocational training, and additional recovery resources / meetings.
7. Assist patients with transportation to medical and other related appointments.
8. Prepare and maintain written records as needed by the program.
9. Participate in all required administrative and training meetings in order to ensure adherence to policies and protocols.
10. Provide emotional and physical support throughout the perinatal period
11. Educate participants on healthy perinatal behaviors and care recommendations, reinforcing messages from healthcare providers.
12. Support partner / father involvement in perinatal care and infant-related activities.
13. Conduct regular postpartum depression screenings during clinic visits.
14. Collaborate with certified lactation consultants to ensure participants receive lactation education and assistance.
15. Provide resources and guidance for breastfeeding, including assistance with equipment, positioning, and
addressing feeding challenges.
16. Be on-call for labor and delivery support as needed.
Qualifications
- Any one, or any combination that totals 3 years from the following :
A. college course work comparable to that leading to a major in social work or one of the social or behavioral sciences or closely related field as measured by the following conversion table or its proportional equivalent :
30 semester hours equals one (1) year (12 months)
B. progressively responsible social service / counseling work experience at a paraprofessional level under qualified supervision
2. Certified Doula or willingness to complete Doula training within 6 months of hire.
Condition of Employment : Pursuant to the State Universities Civil Service System, an out-of-state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date
