- Bi-monthly Home Visits to your clients
- Create goals and monitor progress toward goals
- Create schedules
- communicate with the state , client , families and caregivers.
- Previous experience as a Care Coordinator or in a similar position is preferred
- Certification as a medical assistant is preferred
- Strong problem-solving and organizational skills
- Ability to manage multiple projects or tasks and prioritize appropriately
- Ability to work in fast-paced situations and make sound decisions quickly
- Excellent interpersonal skills and high level of compassion
- Strong verbal and written communication skills
- Must Drive
- Bilingual

You will work closely with our clients and their families to develop personalized care plans that meet their needs and preferences. Your role will be critical in ensuring our clients receive the highest quality of care possible.
